Documents to be submitted to co-op management company:
1. Complete Application in its entirety (sign and date where indicated). If all the
information is not provided, it will delay the processing of the application.
2. Credit check signed and dated by each sublettor
3. Proof of income
4. Complete Tax returns for the past two (2) years including all
schedules (Federal and State)
5. Statement of Financial Condition (form enclosed), copies of records,
statements or agreements to support this statement, copies of the last three (3)
bank statements
6. Copy of anticipated lease and lead Based forms, window guards form, visual
inspection form, bedbug rider and carbon/smoke forms executed by all parties.
7. Verification of residence letter from landlord/management as to the length of
time of your residence and payment history
8. Three (3) letters of reference (can not be from relatives) for each occupant 18
years of age or older
9. Photo Identification for each individual 18 years of age or older
10. $350.00 non refundable processing fee made payable to building mgmt company and $50.00 per applicant made payable to Management com
Only one applicant will be chosen to apply at a time. Because it takes some time for an applicant to gather required documents and receive board approval, signing a letter of intent with good-faith deposit equal to half-month's rent is required (fully refundable if application is completed but not approved; otherwise, to be credited to first month's rent)
