Dear Applicant, Thank you for considering renting my place. An application to rent is available upon request or the Zillow application can be provided. My current tenant screening criteria are as follows: Rental History Each applicant must demonstrate a history of meeting all prior lease or mortgage obligations and leaving any prior rented properties in good order. Significant adverse rental history will disqualify an applicant. Income History Wages, assets, or subsidy assistance are accepted as evidence of ability to pay rent. If household income does not equal at least three times (3x) the monthly payment for which they are responsible, a co-signer, guarantor, or additional security deposit is required. Applicants must include two (2) pay stubs and/or bank statements from the past ninety (90) days with their application. Wages, assets, and/or subsidy assistance are accepted as evidence of ability to pay rent. If household income excluding subsidies does not equal at least three times (3x) the monthly payment after subsidy assistance, a co-signer, guarantor, or additional security deposit is required. Applicants must include two (2) pay stubs and/or bank statements from the past ninety (90) days with their application. Credit History TransUnion's ResidentScore is used to evaluate the applicant's credit history and ability to pay rent when due. A nonrefundable fee equal to the cost is charged to each applicant. A co-signer, guarantor, or additional security deposit is required where the applicant's credit history falls in the 'medium risk' range. An applicant will be declined if their credit history indicates high risk. To avoid a credit history fee, applicants can provide a reusable tenant screening report with the following characteristics, information, and statement: 1. must be in writing; 2. prepared by a consumer credit reporting agency; 3. prominently states the date through which the information contained in the report is current; 4. the name of the applicant; 5. the contact information for the applicant; 6. a verification of source of income of the applicant; 7. the last known address of the applicant; 8. the results of an eviction history check of the applicant in a manner and for a period of time consistent with applicable law related to the consideration of eviction history in housing; 9. the report was prepared within the previous 30 days by a consumer credit reporting agency at the request and expense of a prospective tenant; 10. the report is made directly available to a landlord for use in the rental application process or is provided through a third-party website that regularly engages in the business of providing a reusable tenant screening report and complies with all State and federal laws pertaining to use and disclosure of information contained in a consumer report by a consumer credit reporting agency; 11. the report is available to the landlord at no cost to access or use; 12. the report includes all of the criteria consistently being used by the landlord in the screening of prospective tenants; and 13. applicant must also state there has not been a material change to the information in the reusable tenant screening report. Criminal History A criminal background check is needed of all applicants. A nonrefundable fee equal to the cost is charged to the applicant. If criminal history is found, an individualized assessment of the applicant's criminal history will be conducted. An applicant has the right to dispute inaccuracies relevant to criminal history and to provide evidence of rehabilitation or other mitigating factors related to their criminal background. See the Cook County Commission on Human Rights' at 69 W. Washington Street, Suite 1130, Chicago, Illinois 60602. Will Not Rent To Any Of The Following: 1. Smokers; including users of tobacco, e-cigarettes, vaping, and cannabis. 2. Those under a current sex offender registration requirement pursuant to the Sex Offender Registration Act (or similar law in another jurisdiction). 3. Those under a current child sex offender residency restriction. 4. Applicants who do not respond by the next business day to communications. A response includes a reply requesting additional time as long as the reply states when the Applicant will fully respond and the Applicant actually provides a complete and timely response. 5. Applicants who use the name, account, and/or identity of a different person or a fictitious person when contacting me through an online home rental service. 6. Applicants who (a) do not meet for a showing and (b) do not provide at least two hours notice they are not able to meet for the showing. 7. Applicants who (a) are more than ten minutes late to a scheduled showing and (b) do not provide their estimated arrival time before ten minutes after their scheduled showing time. Please Attach All Of The Following: 1. A copy of the front and back of your current driver's license or state identification card. 2. A credit report from one of the three credit reporting bureaus dated from the past 90 days. A free credit report can be obtained at annualcreditreport website. 3. The last two pay stubs and/or bank account statements from the past 90 days to prove your income. 4. The names and phone numbers of the following references: a. Your manager or supervisor at work who has direct knowledge of your work performance and salary. b. Your current landlord, landlord's agent, or management company representative who has direct knowledge of any issues with your tenancy and your rent payment history. Fees, Costs, And Deposits: 1. I do not charge a fee for reviewing an application, but I request reimbursement for all out of pocket costs including credit reports, background checks, and employment verifications. A reusable tenant screening report meeting the above requirements is acceptable. 2. Before any costs are incurred, an initial review is performed. If the result is favorable, the applicant is notified the next step is to order a TransUnion SmartCheck Basic report or provide a reusable tenant screening report. If the applicant agrees, the report is ordered and the costs are reimbursed by the applicant. The cost as of February 20, 2026 is $25.00 per screening. One screening is needed for each applicant. If there are co-signers and/or guarantors, one or more people will need to be screened, depending on the results. 3. Below are the condominium association fees and management company fees as of February 20, 2026. Fees may change without notice. Condominium association lease processing fee: $350. No charge for renewals. Management company lease processing fee: $175. Renewals cost $75. Condominium association moving fees: Move-in fee: $500 on weekdays, $750 on Saturdays. Move-in refundable damage deposit: $350. Move-out fee: $500 on weekdays, $750 on Saturdays. Move-out refundable damage deposit: $350. 4. Renters insurance is required with the following policy minimums: Personal liability coverage must be at least $300,000. Personal property coverage has no minimum.
